Before you begin:

1. Refer to your Information Request letter for your Premises Number. You will need it for the questionnaire.
2. Grab a coin to scratch the surface of the pipe and a fridge magnet if you have one.
3. Identify your Service Line Material Type. See sample photos on this page.

Complete the questionnaire:

1. Locate your water service line in your basement or utility closet.
2. Click on the link below for the Service Line Material questionnaire.
3. Add your Premises Number and fill out a few quick questions.
4. Snap a photo of your Water Service Line and upload.
5. Check the Captcha box.
6. Click Submit and you are done!
